For FLAT RATE Rigid Scope/s being sent for repair, please follow these steps:

  1. Enter your scope information on the form below, INCLUDE MANUFACTURER, MODEL, SERIAL NUMBER & DEGREE
  2. Check your email for your packing slip for a PACKING SLIP and PREPAID SHIP LABEL do not include additional paperwork
  3. Be sure to disinfect all medical devices
  4. Send the equipment previously authorized matching with the packing slip in an appropriately sized box
  5. Be certain to securely pack the item/s being sent in for repair NO SHAKE, RATTLE OR ROLE
  6. All scope repairs must be prepaid.
