1. Preparation & Packing

• Disinfect: Thoroughly sanitize all devices before packaging.

• Pack Securely: Box items carefully with adequate cushioning to avoid damage in transit.

• Include Form: Print this page/email and place a copy inside the package.

3. Evaluation & Payment Terms

• Evaluation Fee: A standard $89 fee applies per scope, which is waived if you approve the repair quote.

• Declined Repairs: If you decline the quote, the $89 fee plus return shipping costs must be paid prior to returning the equipment.

• Prepayment & Disposal: Full payment is required before repairs begin. Unpaid scopes will be discarded after 30 days
